Acceptable forms of identification are:

• state-issued driver’s license

• passport issued by the US Department of State

• state-issued identification card

• US military ID

• state, county, or local government ID

• permanent resident card or “green card issued by the US Citizenship & Immigration Services

• inmate ID issued by State Department of Corrections (or Federal Bureau of Prisons, if the inmate is in state or federal custody.)

Failure to present valid and current identification at the time of the appointment will result in a non-refundable cancellation of the appointment, and a non-negotiable fee of $25.00 will be charged to you or your client to cover administrative costs.
